Team Agile Maturity Assessment
This document outlines a structured way to assess Agile maturity within a team using a simple, objective questionnaire and a supporting scoring model. The intent is not to enforce a specific framework, but to provide a consistent way of understanding how independently a team can operate, where it may still rely on external support, and how that support should evolve over time.
The questionnaire is designed to be answered by any team member and focuses on practical behaviours such as decision-making, ownership, problem resolution, and continuous improvement. Rather than measuring theoretical knowledge of Agile, it looks at how the team actually operates day to day. The scoring model then translates those responses into a maturity level, giving a clear indication of whether the team is fully supported, operating with guidance, or functioning as a self-organising unit.
Beyond the scoring itself, this document also provides guidance for management on how to interpret results over time. This includes how to combine quantitative scores with qualitative feedback, how to align support levels with observable outcomes such as delivery quality and predictability, and how to adjust coaching involvement based on real team performance. The goal is to create a feedback loop where support is applied where it is needed, and removed where it is no longer required, allowing teams to grow into sustainable independence.
Questionnaire
This is a yes/no questionnaire that can be used to objectively determine the level of support a team is in need of from an external Agile Coach and can be asked to any team member. The questions can be displayed randomly and should not be presented with the internal heading, the heading is only for scoring where potential support might be needed for a specific team.

Can your team successfully run Agile events (e.g., retrospectives, planning sessions) without the involvement of an Agile coach?
Does the team generally follow Agile processes while also making necessary adjustments on their own?
Continuous Improvement
Does the team regularly identify action items during retrospectives without prompting from an Agile coach?
Are these action items consistently implemented and tracked without relying on an Agile coach?
Problem Resolution and Blockers
Does the team proactively resolve most challenges and blockers before consulting an Agile coach?
Is the team able to identify issues that would require escalation outside of the team?
Team Ownership and Initiative
Do team members own their work and drive product/project progress with minimal guidance from the Product Owner or Agile coach?
Does the team effectively prioritize and organize work without needing assistance of an Agile coach?
External Dependency
Could the team operate effectively for at least a month without support of an Agile coach?
Does the team confidently adjust Agile practices to suit evolving needs without consulting an Agile coach?
Self-Organizing Practices
Does the team independently refine Agile practices to fit their workflow?
Do team members regularly exchange feedback on Agile practices and their effectiveness?
Team Trust
Do you feel comfortable expressing your thoughts, opinions, and feedback during Agile events?
Do team members openly share feedback and ideas in both formal meetings and informal discussions?

No = 0 points
For each question, team members receive either 1 or 0 points, depending on their answer. The points are totalled and then averaged across team members to get an overall score for the team.
Note: A team must score a minimum of 1 point on average for each category to move above Support with Regular Guidance. There is also a need for an Agile Facilitator to reach the stage of Supported Independence.
Support Levels and Score Ranges

How to Use the Scale
Calculate Total Score: For each team member, add up the points from their answers.
Average Team Score: Calculate the average score across all team members to get an overall team score.
Map to Support Level: Compare the team’s average score to the score ranges above to determine the support level.
Example Calculation
Individual Scores: 14, 16, 13, 17, 18
Team Average: (14 + 16 + 13 + 17 + 18) / 5 = 15.6
Support Level: The team average of 15.6 maps to Coaching and Mentoring.
Quarterly Scoring Surveys: Use the scoring system outlined above to survey team members quarterly. Analyse scores to see if the team’s maturity level aligns with the expected level of support. If a team consistently scores higher or lower than their assigned support level, this may indicate they need more or less support.
Check-In with Agile Coach and SEMs: Gather feedback from Agile coaches and SEMs to get qualitative insights on the team's maturity and independence in Agile practices.
